ADMS Pathways was created to help individuals and families navigate important life transitions with guidance, advocacy, and practical support. Many families face overwhelming systems, limited resources, and uncertainty when trying to plan for the future. Our goal is to help bridge those gaps and provide a clearer path forward.

We believe transitions should not feel confusing or isolating. Whether a young person is preparing for adulthood, a family is trying to understand available supports, or an individual is working toward greater independence, ADMS Pathways is here to provide structure, encouragement, and connection to meaningful opportunities.
